It helps to have the following information ready when you submit a claim.
If you submit a claim by fax, be sure to include this information in your fax:

• Your policy number
• The date the loss happened
• The policyholder's name and address
• A description of what happened
• If possible, please include a telephone number where we can contact you

When you fill out and submit our online claim form, whether it's for Foremost, Zurich, or American Federation, we'll document the information on our system. A member of our claim staff will contact you to let you know what will happen next.
